Our Mission
For over 20 years, Autumn Grace Medical Associates has worked to help immigrants through the naturalization process while providing affordable and accessible healthcare. With an office of multilingual staff members, some of whom are naturalized immigrants themselves, we strive to provide patients with assistance and guidance in the naturalization process, as well as comprehensive medical attention, all at affordable prices.
What Will Occur?
1) Book your drop-in either by giving us a call with a general time frame, or selecting a time below. (As long as it’s before closing and not 12pm-1pm, we’re very flexible with time.)
2) At the drop-in, we will review the paperwork and set you up for your first of 2 visits with Dr. Jonathan Chih Yeh.
3) The first visit will be by the phone. Dr. Jonathan Chih Yeh will call you for a general exam and to send any lab orders to you.
4) The second visit will occur in the office, where you will be given any required vaccines and the completed paperwork.
5) You’re all done!

Patient portion must be completed according to instructions prior to appointment with black ink & no errors.
Government -Issued ID
We accept either a Passport or a Driver’s License
Labs and Immunization Records
Please bring any available documents. Having these documents ready may reduce the cost of your visit.
